You can easily swap gems in items since Season 6, i.e. since October 2024. Previously, this was not possible and you had to unsocket the socketable items at the jeweler, which was especially annoying for players who liked to experiment and test different builds.

With the new runes, Season 6 also brought the practical function of being able to exchange gems. You simply drag the desired stone from your inventory onto the item, and the old one automatically ends up in your inventory.
